Assertive Community Outreach Team

Working Towards Housing for 100 Chronically Homeless Individuals in Utah

The Assertive Community Outreach Team (ACOT) at Volunteers of America, Utah works with chronically homeless individuals who have serious mental illness and/or substance abuse disorders. It is a multi-disciplinary team that provides case management, vocational services, behavioral health treatment and recovery-oriented services to enable individuals to succeed in housing. Each client receives consistent, weekly services from the team’s mental health therapists, case manager, vocational specialist and psychiatrist. The goal of the program is to get 100 of these individuals housed and fully engaged in improving their lives within three years—meaning they have jobs and/or are using mainstream benefits.
