Case Management Services
The process of transition from living on the street and/or in shelter to living in one’s own apartment is a big change for many of our clients. As an agency we have learned that the transition often works best when our clients have someone they can rely on. The Homeless Outreach Program provides case management at a number of locations in our community. Our case managers provide home-based supportive services to formerly homeless individuals. Locations where case management is provided include: the Pathways Project (scattered site housing) ,Grace Mary Manor, Palmer Court. These projects are a collaboration of numerous agencies within our community including the Salt Lake County Housing Authority, The Road Home, the Housing Authority of Salt Lake City, and Valley Mental Health.
