The Homeless Outreach Program delivers mobile street outreach to homeless individuals and families in Salt Lake County. Program outreach workers seek out the homeless on the streets, under viaducts, along railroad tracks and along the banks of creeks. Once found, the team offers basic survival items, food, sleeping bags, coats, and other necessities. Through consistent contact, the outreach team seeks to build a basic level of trust and assess the needs of those served. Outreach workers offer referrals and, if accepted, will transport individuals to needed community services. Individuals who require longer term services are referred to the program Care Coordinator for follow up case management.

Pathways Project

In August of 2005, the Pathways Project was established to test the Housing First strategy with the chronically homeless in Salt Lake County. The project is following 17 individuals who, after three or more years of persistent homelessness, were placed into apartments. In addition to housing, participants receive intensive case management services. To date, 16 of the 17 individuals have been continuously housed. Prior to the Pathways Project, more than 985 days of detoxification services were utilized by 9 participants.  Since the project commenced, this number has been reduced to 6 clients utilizing 35 days, with a reduction in costs to the community of over $55,000.
